How do you assess a child educational level?

What tools can I use to determine a teen’s grade level skills? The last grade she completed was 9th grade. But she can’t even write a complete sentence. I don’t know what level she is with reading comprehension either. Are there any books or websites to help me assess her educational level so I can begin to tutor her.

    Okay I would really like to help but I’ve never done diagnostic assessment before, I just know what it is. Why don’t you get help from the experts. Go to sites where many tutors & retired teachers hang out:
